29 CFR  1926.1053(b)(1):Where portable ladders were used for access to upper landing surfaces, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface to which the ladders were used to gain access:  On or about September 18, 2024, and times prior to, the employer did not ensure an extension ladder employees used to access the second level extended 3 feet above landing, exposing them to fall hazards greater than 6 feet.
