29 CFR 1926.652(a)(1): Each employee in an excavation was not protected from cave-ins by an adequate protective system designed in accordance with 29 CFR 1926.652 (b) or (c). a) 11 Douglassville Shopping Center (rear), Douglasville, PA, Worksite - On or about September 18, 2024, an employee worked in a straight cut trench approximately 6 feet deep without cave-in protection, exposing them to an engulfment hazard. Abatement certification required within 10 days after abatement date. The certification shall include a statement that abatement is complete, date and method of abatement, and state employees and their representatives were informed of this abatement.

29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed: a) 11 Douglassville Shopping Center (rear), Douglasville, PA, Worksite - On or about September 18, 2024, an employee accessed a straight-cut trench approximately 6 feet deep by a ladder for a safe means of egress that did not extend 3 feet above the upper landing surface, exposing the employee to a fall hazard. Abatement certification required within 10 days after abatement date. The certification shall include a statement that abatement is complete, date and method of abatement, and state employees and their representatives were informed of this abatement.
