On or about September 17, 2024, at the above addressed jobsite, an employee using a pneumatic nail gun was not wearing safety glasses, thereby exposing the employee to the hazards associated with eye injuries.

On or about September 17, 2024, at the above addressed jobsite, employees performing residential roofing operations, fourteen (14) feet nine (9) inches, above the ground, were not protected from falls, thereby exposing employees to fall hazards.  b.  On or about September 17, 2024, at the above addressed jobsite, an employee performing residential roofing operations, nine (9) feet four (4) inches, above the ground, was not protected from falls, thereby exposing the employee to fall hazards.  c.  On or about September 17, 2024, at the above addressed jobsite, an employee performing residential roofing operations, seven (7) feet eight (8) inches, above the ground, was not protected from falls, thereby exposing the employee to fall hazards.

On or about September 17, 2024, at the above addressed jobsite, employees conducting roofing operations were not provided training on how to minimize fall hazards, thereby exposing the employees to the hazards associated with falls.
