29 CFR  1926.20(b)(2): The employer did not initiate and maintain programs which provided for frequent and regular inspections of the job site, materials and equipment to be made by a competent person(s):  649 Chestnut St., Pottstown, PA - Roof and associated work areas: On or about September 17, 2025, various jobsite hazards existed in plain sight and exposed employees to injury. An inspection of the job site was not conducted by a competent person.

29 CFR  1926.501(b)(13):Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):  649 Chestnut St., Pottstown, PA - Roof and associated work areas: On or about September 17, 2024, an employee was exposed to fall hazards of 20 feet or more while performing work activities from multiple locations on a  residential roof. No personal fall protection system or other fall prevention measures were utilized.
