29 CFR  1926.652(a)(1): Each employee in an excavation was not protected from cave-ins by an adequate protective system designed in accordance with 29 CFR 1926.652 (b) or (c):  (a)NorthWestern Energy Group, Inc. at SD 37 near the intersection of North Main Street and 23rd Avenue in Mitchell, SD 57301:  On or about and at times prior to September 18, 2024, the exposing employer did not ensure that employees conducting gas-line connection operations within an excavation greater than five feet in depth were protected from excavation wall cave-in by an adequate protective system.   This condition exposed employees to struck-by and cave-in hazards associated with an excavation wall collapse.
