29 CFR 1926.502(d)(15): Anchorages used for attachment of personal fall arrest equipment were not capable of supporting at least 5,000 pounds per employee attached. Location: 38 Crown Road, Westford, MA; On or about 09/19/2024, employees were allowed to utilize temporary roof anchors which were not installed per manufacturer instructions to support 5,000lbs per employee in the following instances: a) Anchorage point on mid section of residential roof was not secured per manufacturer instructions and had multiple missing fasteners and was lifting off roof surface. b) Anchorage point on the right end of residential roof furthest from the driveway and at the front of residence was not secured per the manufacturer instructions and had multiple missing fasteners.
