29 CFR  1910.28(b)(1)(i): The employer did not ensure that each employee on a walking-working surface with an unprotected side or edge that is 4 feet (1.2m) or more above a lower level was protected from falling by guardrail systems, safety net system, or personal fall arrest system:  At 929 SW University Blvd, Topeka, Kansas, employees were exposed to fall hazards greater than 4 feet to the ground level below. Employees were not protected from fall hazards by means of a guardrail, netting, or other fall protection system when storing and removing materials from storage racks at 12 feet in height.
