29 CFR 1910.212(a)(1): One or more methods of machine guarding shall be provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks. Examples of guarding methods are-barrier guards, two-hand tripping devices, electronic safety devices, etc. On or about and at times prior to October 2, 2024, at the establishment located at 1100 West Mason Street, Green Bay, Wisconsin; in the New Brush Area, the employer did not ensure that equipment was adequately guarded, exposing employees to contact with the point of operation of machines in the following instances: a) Brush Machine 4 (Mfr: Anton Zahoransky, Type: E205, Nr. 2000331) was not adequately guarded at the point of operation during operation. b) Brush Machine 5 (Mfr: Anton Zahoransky, Type: E205, Nr. 2000362) was not adequately guarded at the point of operation during operation.

29 CFR 1910.1200(e)(1): Employers shall develop, implement, and maintain at each workplace, a written hazard communication program which at least describes how the criteria specified in paragraphs (f), (g), and (h) of this section for labels and other forms of warning, safety data sheets, and employee information and training will be met. On or about and at times prior to October 2, 2024, at the establishment located at 1100 West Mason Street, Green Bay, Wisconsin; at Pad Print, the employer did not ensure that their written hazard communication program was implemented when employees worked with hazardous chemicals in the workplace, such as but not limited to paint thinner, exposing employees to chemical hazards. 29 CFR 1910.1200(f)(6): Except as provided in paragraphs (f)(7) and (f)(8) of this section, the employer shall ensure that each container of hazardous chemicals in the workplace is labeled, tagged or marked with either: (i) The information specified under paragraphs (f)(1)(i) through (v) of this section for labels on shipped containers; or, (ii) Product identifier and words, pictures, symbols, or combination thereof, which provide at least general information regarding the hazards of the chemicals, and which, in conjunction with the other information immediately available to employees under the hazard communication program, will provide employees with the specific information regarding the physical and health hazards of the hazardous chemical. On or about and at times prior to October 2, 2024, at the establishment located at 1100 West Mason Street, Green Bay, Wisconsin; at Pad Print, the employer did not ensure that each container of hazardous chemical was labeled, tagged or marked, with the identity and/or hazards of its contents, exposing employees to chemical hazards in the following instances: a) One plastic bottle that contained paint thinner was not properly labeled, tagged or marked. b) One metal plunger can that contained paint thinner was not properly labeled, tagged, or marked.
