29 CFR 1926.1053(b)(16):  Portable ladders with structural defects, such as, but not limited to, broken or missing rungs, cleats, or steps, broken or split rails, corroded components, or other faulty or defective components, shall either be immediately marked in a manner that readily identifies them as defective, or be tagged with "Do Not Use" or similar language, and shall be withdrawn from service until repaired.  On or about the week of September 14, 2024 and thereafter, at various jobsites in Wisconsin;  (a)  Employee(s) utilized the Keller KPro 6ft Type IA Fiberglass Twin Ladder while performing gutter and downspout installation activities.  The ladder was not removed from service due to the spreader bar was bent allowing the rivets securing it on the side rails to move exposing employees to fall hazards.  (b) Employee(s) utilized the Little Giant Safe Frame 10ft Stepladder while performing gutter and downspout installation activities.  The ladder was not removed from service due to the seventh rung from the bottom brace was missing a rivet exposing employees to fall hazards.  (c)  Employee(s) utilized the Keller KPro 6ft Type IA Fiberglass Twin Ladder while performing gutter installation activities.  The ladder was not removed from service due to the second and fourth rungs were bent exposing employees to fall hazards.  (d) Employee(s) utilized the Keller KPro 6ft Type IA Fiberglass Twin Ladder while performing gutter installation activities.  The ladder was not removed from service due to the spreader bar being bent and the rivets used to secure it was loose exposing employees to fall hazards.
