This condition exposed employees to fall hazards.

29 CFR  1926.100(a): Employees working in areas where there was a possible danger of head injury from impact, or falling or flying objects, or from electrical shock and burns, were not protected by protective helmets:  (a)  On or about October 1, 2024, an employee on the ground conducting cleanup of roofing materials at an apartment building located at N168 W20563 HWY 60 in Jackson, Wisconsin, was not wearing a hard hat while working below the workers performing roofing work. This exposed the employee to struck-by hazards from tools and materials.

29 CFR  1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):   (a)  On or about October 1, 2024, employees conducted re-roofing work on an apartment building at N168 W20563 HWY 60 in Jackson, Wisconsin, without the use of fall protection. This exposed employees to fall hazards of approximately 17 feet 5 inches at the upper roof and approximately 11 feet at the lower roof.

29 CFR  1926.1053(b)(1): Portable ladders were used for access to an upper landing surface and the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface to which the ladder was used to gain access:  (a)  On or about October 1, 2024, employees conducted re-roofing work on a multifamily building, at N168 W20563 HWY 60 in Jackson, Wisconsin, utilized a ladder for access/egress to a roof area that extend approximately one (1) foot above the landing, exposing an employee to a fall hazard of approximately 17 feet 5 inches.
