29 CFR  1926.100(a):Employees working in areas where there was a possible danger of head injury from impact, or falling or flying objects, or from electrical shock and burns, were not protected by protective helmets:  On or about October 1, 2024, employees engaged in residential roofing activities, located at 9110 Woolworth Ave, Omaha, NE 68124, were exposed to struck-by hazards in that at least one employee was working below an existing roof without being protected from falling objects while workers above them were throwing debris from the 11-foot 11-inch high roof.

Each employee engaged in roofing activities on low-slope roofs, with unprotected sides and edges 6 feet (1.8 m) or more above lower levels, was not protected from falling by guardrail systems, safety net systems, personal fall arrest system.  On or about October 1, 2024, employees engaged in residential roofing activities, located at 9110 Woolworth Ave, Omaha, NE 68124, were exposed to fall hazards in that the employees were working in excess of 6-feet while conducting residential roofing operations without fall protection.
