29 CFR  1926.651(c)(2):A stairway, ladder, ramp or other safe means of egress was not located in trench excavations that were 4 feet (1.22m) or more in depth so as to require no more than 25 feet (7.62m) of lateral travel for employees:  On or about October 1, 2024, on the north side of a commercial parking lot, employees working inside an excavation without having a ladder available for egress.

29 CFR  1926.651(e):Exposure to falling loads. No employee shall be permitted underneath loads handled by lifting or digging equipment. Employees shall be required to stand away from any vehicle being loaded or unloaded to avoid being struck by any spillage or falling materials. Operators may remain in the cabs of vehicles being loaded or unloaded when the vehicles are equipped, in accordance with 1926.601(b)(6), to provide adequate protection for the operator during loading and unloading operations:   On or about October 1, 2024, on the north side of a commercial parking lot, an employee was working underneath an excavator's arm with a suspended load.

On or about October 1, 2024, on the north side of a commercial parking lot, the spoil piles were located less than 2 feet from the edge of the excavation walls.
