29 CFR  1926.252(a): An enclosed chute of wood, or equivalent material, was not used where materials were dropped more than 20 feet to points lying outside the exterior walls of the building(s).  Worksite: On or about 10-02-2024, ground worker employees were exposed to struck-by hazards while working approximately 24 feet below roof workers who were removing, installing, and dropping roofing materials, utilizing only mesh tarps to direct falling debris to the ground, without an enclosed chute of wood, or equivalent material.

29 CFR  1926.501(b)(1): Each employee on a walking/working surface having an unprotected side or edge which was six or more feet (1.8m) or more above a lower level was not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest system.  Worksite: On or about 10-02-2024, employees were exposed to fall hazards while removing and installing roofing material at a height of approximately 24 feet above the lower level without any means of fall protection.

(a) Worksite: On or about 10-02-2024, employees gained access to and from a roof surface while utilizing a 40-foot Werner extension ladder with its two rails set unequally on the arched portion of a pitched roof, causing it to be unstable, thus exposing employees to fall hazards.
