29 CFR 1910.178(l)(2)(ii): The employer did not ensure that each operator had successfully completed the training consisting of a combination of formal instruction (e.g., lecture, discussion, interactive computer learning, video tape, written material), practical training (demonstrations performed by the trainer and practical exercises performed by the trainee), and evaluation of the operator's performance in the workplace.  a. On or about October 02, 2024, at 12330 49th Street North, Clearwater, FL 33762, employees were operating a forklift to lift/move outboard boat engines and to move/towe boats on trailers, during which the employer failed to ensure that the operators of the forklifts had received initial operator training, in that, the operators had not been trained by an "Experienced" person, nor had the operators training consisted of formal instruction, practical training, and an evaluation of the operator's performance in the workplace; therefore, exposing the operator and the employees in the work area/pedestrians to a roll-over/crushed-by, fall-from and struck-by hazard(s).

On or about October 02, 2024, at 12330 49th Street North, Clearwater, FL 33762, employees were operating a forklift to lift/move outboard boat engines and to move/towe boats on trailers, during which time the employer failed to ensure that the forklift operators initial training, instruction and evaluation and triannual evaluation was conducted by a "Experienced" Person/Trainer (by persons who have the knowledge, training, and experience to train powered industrial truck operators and evaluate their competence); therefore exposing the operator and the employees in the work area/pedestrians to a roll-over/crushed-by, fall-from and struck-by hazard(s).
