On or about October 02, 2024, at 12330 49th Street North, Clearwater, FL 33762, employees were operating a forklift to lift/move outboard boat engines and to move/towe boats on trailers, during which the employer failed to ensure that the operators of the forklifts had received initial operator training, in that, the operators had not been trained by an "Experienced" person, nor had the operators training consisted of formal instruction, practical training, and an evaluation of the operator's performance in the workplace; therefore, exposing the operator and the employees in the work area/pedestrians to a roll-over/crushed-by, fall-from and struck-by hazard(s).
