29 CFR  1910.140(c)(13)(ii):Anchorages were not designed, installed, and used, under the supervision of qualified person, as part of a complete personal fall protection system that maintains a safety factor of at least two:  (a): 171 Dundee Avenue, Paterson NJ Backyard West-Side - An employee was exposed to fall hazards of approximately 15 feet while performing tree trimming work using an anchorage point on the tree that was not determined to be capable of supporting at least two times the climber's weight . The employee suffered a fatal fall when the branch he was secured to broke, on or about October 2, 2024.  There is no abatement certification or documentation required for this item.
