29 CFR  1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks:  a) On or about 10/03/2024, in the Press Brake Area;  Employees were exposed to amputation hazards when Hydraulic Bender Roto-Die Press Brake Model 15 Serial #98845 point of operation was not guarded.  Metal bending operations were performed when the EZ-Screen Banner light curtains were improperly installed above the point of operation such that the light curtain did not guard the point of operation.   ABATEMENT DOCUMENTATION REQUIRED
