A) 11 Maplewood Road, Scarsdale - NY: An employee was exposed to fall hazards of approximately 11 feet while working on a pump jack scaffold that was not braced at the bottom. The employee was not wearing any type of fall protection either, on or about, October 3rd, 2024.

A) 11 Maplewood Road, Scarsdale - NY: Employees were exposed to fall hazards of approximately 12 1/2-feet while engaged in roofing work on a structure, without a means of fall protection in place, on or about, October 3rd, 2024.
