On or about October 8, 2024, an employee of Massillon Cable TV, Inc. dba MCTV, Inc. was working from the boom or basket of a vehicle mounted articulating boom platform (aerial lift) and was not wearing a personal fall arrest or travel restraint system while installing fiber optic cable on utility poles located at 4614 Manchester Ave, SW, Navarre, Ohio 44662.

The employer shall ensure that no employee approaches or takes any conductive object closer to any electrically energized overhead power lines and parts than prescribed in Table R-2, unless:  On or about October 8, 2024, an employee of Massillon Cable TV, Inc. dba MCTV, Inc. working as a fiber installer, did not maintain an approach distance of 24 inches or greater (as prescribed in 1910.268 Table R-2) of an electrically energized 7,200 volt overhead power line while installing fiber-optic cable on utility poles located at 4614 Manchester Ave, SW, Navarre, Ohio.

These devices shall not be operated with any conductive part of the equipment closer to exposed energized power lines than the clearances set forth in Table R-2 of this section.  On or about October 8, 2024, an employee of Massillon Cable TV, Inc. dba MCTV, Inc. was operating an uninsulated vehicle-mounted elevating and rotating articulating work platform (boom truck)  and did not maintain an approach distance of 24 inches or greater (as prescribed in 1910.268 Table R-2) of an electrically energized 7,200 volt overhead power line with the conductive part of the equipment (basket) while installing fiber optic cable on utility poles located at 4614 Manchester Ave, SW, Navarre, Ohio.
