29 CFR 1910.111(b)(12)(vii): Loading and unloading systems was not protected by suitable devices to prevent emptying of the storage container or the container being loaded or unloaded in the event of severance of the hose: a) On or about October 8, 2024, at the NC-201 Column, employees engaged in neutralizing acid operations utilized a 150 lb. Anhydrous Ammonia bottle that was not equipped with suitable devices to prevent emptying of the storage container in the event of severance of the hose, thereby exposing employees to the release of Anhydrous Ammonia, a Hazardous Material. Backflow check valves or properly sized excess flow valves were not installed where necessary to provide such protection.
