The spoils pile was placed at the edge of the excavation on the South face.  This condition exposed employees to struck-by hazards.

29 CFR 1926.652(g)(1)(ii): Shields were not installed in a manner to restrict lateral or other hazardous movement of the shield in the event of the application of sudden lateral loads:  (a)	NW side of 118th Ave and Tower Rd, Commerce City, CO 80022: On or about 10/8/2024, and at times prior, the employer did not ensure that each employee working inside an excavation that was approximately 17 feet in depth was protected from cave-in.  The trench box at the East end of the excavation was placed in such a manner as to not restrict its lateral movement in the event of a collapse of the South face.  The South face was sloped at approximately 50 degrees, had fissures, signs of sloughing, spoils placed at the edge, and was not flush with the side panel of the box.  Submerged water was located at the base of the small trench.  This condition exposed employees to struck-by hazards.
