29 CFR 1926.501(b)(13): Each employee engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system.  On or about October 10,  2024, at the front side of a one story home, roofers were performing roofing operations at a height greater than six feet without the use of fall protection while they were replacing a damaged roof.

29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed.  On or about October 10, 2024, at the front right side of the home, roofers were gaining access to the roof in which the side rails of the ladder were not at least three feet above the landing surface.
