Tri County Transportation Incorporated, 662 Airport Road, Punxsutawney, PA 15767, on or about October 15th, 2024 and times prior thereto: The recordable injury and/or illness entries on the OSHA 300 Log for the Calendar Year 2024 were not completed as required.  In Column F the recordable cases that occurred on 1/12/2024 through 10/9/2024 were not descriptive, in that the body part affected, and/or the agent, object, and/or substance that directly injured or made the person ill was not noted/documented.

Tri County Transportation Incorporated, 662 Airport Road, Punxsutawney, PA 15767, on or about October 15th, 2024 and times prior thereto: Fire extinguishers were not identified so that they were readily accessible. Areas surrounding fire extinguishers contained no signage or labeling indicating the presence of a fire extinguisher.

29 CFR  1910.1200(f)(6)(ii): Except as provided in 29 CFR 1910.1200(f)(7) and 29 CFR 1910.1200(f)(8), the employer did not ensure that each container of hazardous chemicals in the workplace was labeled, tagged or marked with the product identifier and words, pictures, symbols, or combination thereof, which provide at least general information regarding the hazards of the chemicals and which, in conjunction with the other information immediately available to employees under the hazard communication program, would provide employees with the specific information regarding the physical and health hazards of the hazardous chemical:  a.) Tri County Transportation Incorporated, 662 Airport Road, Punxsutawney, PA 15767, on or about October 15th, 2024 and times prior thereto: Secondary containers of products including, but not limited to, Lysol Brand Antibacterial All Purpose Cleaner were not labeled with a product identifier and the hazards of the hazardous chemical(s) container therein.
