29 CFR 1926.25(a): During the course of construction, alteration, or repairs,  debris was not kept cleared from work areas, passageways, and stairs, in and around buildings or other structures.  Bldg. 6005: On or about October 17th, 2024, and at times prior thereto, the employer did not ensure walking-working surfaces were maintained in a safe condition. There was several Guardian Fall Protection Temper Reusable Anchor(s) with protruding nail(s) all over the ground.  Employee(s) were potentially exposed to slip, trip, fall and puncture hazard(s).

Bldg. 6005: On or about October 17th, 2024, and at times prior thereto, the employer did not ensure a fire extinguisher was provided for employee use. Employee(s) used a 5-gallon gas that was used to power a Roofmaster 11HP Honda Single Roof Saw to conduct roofing activities. This condition potentially exposed employee(s) to fire hazard(s).
