29 CFR  1910.67(c)(2)(v):  A personal fall arrest or travel restraint system that meets the requirements of subpart I of this part were not worn or attached to the boom or basket when working from an aerial lift.  a) On or about October 19, 2024, employees operating a vehicle mounted aerial device, International Electrical Bucket Truck, Model MV607 SBA (Truck #3), did not wear a personal fall arrest or use a travel restraint system.  b) On or about October 19, 2024, employees operating a vehicle mounted aerial device, International Electrical Bucket Truck, Model 4300 (Truck #14), did not wear a personal fall arrest or use a travel restraint system.  c) On or about October 19, 2024, employees operating a vehicle mounted aerial device, International Electrical Bucket Truck, Model MV607 (Truck #20), did not wear a personal fall arrest or use a travel restraint system.
