1926.20(b)(2)  The employer did not initiate and maintain programs which provided for frequent and regular inspections of the job site, materials and equipment to be made by a competent person(s)   a) Cherokee Building Materials DBA United Building Material on or about October 21, 2025, at the location of 1008 Fall Ridge Ct. Logan, Bellefontaine Ohio 43311: The employer did not ensure employees were adequately trained in the recognition and avoidance of unsafe conditions related to material handling operations. Employees tasked with the operation of an remote-controlled vehicle-mounted boom without the benefit of this training, were exposed to crushed by hazards while positioned under the boom and between the hoisted load and the bed of the truck.
