29 CFR 1910.1000(a)(2): Employee(s) were exposed to an airborne concentration of carbon monoxide listed in Table Z-1 in excess of the 8 hour Time Weighted Average concentration of 50 parts per million (ppm):  Location: Jobsite, Needham, MA: a. On October 21, 2024, a worker was exposed to an 8-hour time weighted average carbon monoxide level of 96.8 ppm, approximately 1.94 times the OSHA PEL of 50 ppm, where exhaust from a gas-powered generator entered the workspace. The exposure level was calculated based on the % carboxyhemoglobin (COHb) level in the worker's blood.  b. a. On October 21, 2024, a worker was exposed to an 8-hour time weighted average carbon monoxide level of 324.9 ppm, approximately 6.50 times the OSHA PEL of 50 ppm, where exhaust from a gas-powered generator entered the workspace. The exposure level was calculated based on the % carboxyhemoglobin (COHb) level in the worker's blood.  c. On October 21, 2024, a worker was exposed to an 8-hour time weighted average carbon monoxide level of 142.0 ppm, approximately 2.84 times the OSHA PEL of 50 ppm, where exhaust from a gas-powered generator entered the workspace. The exposure level was calculated based on the % carboxyhemoglobin (COHb) level in the worker's blood.

29 CFR 1910.1000(e): Feasible administrative or engineering controls were not determined and implemented to achieve compliance with the limits prescribed in 29 CFR 1910.1000(a) through (d):  Location: Jobsite, Needham, MA: On 10-21-24, administrative and/or engineering controls, including keeping gas powered generators out of indoor workspaces, were not implemented, exposing workers to carbon monoxide in excess of the 8-hour time weighted average of 50 ppm.
