29 CFR 1926.652(g)(1)(iii): Employees were not protected from the hazard of cave-ins when entering or exiting the area protected by shields: (a) Yorktown Shopping Center North Parking Lot Lombard, IL 60148 - On or about October 23, 2024, the employer did not ensure that an employee entered and exited a seventeen-feet deep excavation within the area protected by shields thereby exposing them to cave-in hazards. No abatement certification or documentation is required for this item.

29 CFR 1926.1053(b)(1): Portable ladders were used for access to an upper landing surface and the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface to which the ladder was used to gain access: (a) Yorktown Shopping Center North Parking Lot Lombard, IL 60148 - On or about October 23, 2024, the employer did not ensure that the side rails of a non-self-supporting portable ladder was extended at least three feet above the upper landing surface (pavement) of a seventeen-foot excavation prior to an employee using the ladder to enter and exit the excavation thereby exposing them to fall hazards. Abatement documentation is not required of this item.
