29 CFR 1926.1203(e)(2)(ii): When entrance covers were removed the opening was not immediately guarded by a railing, temporary cover, or other temporary barrier that would prevent an accidental fall through the opening and that would protect each employee working in the space from foreign objects entering the space:  (a) Underground gasoline storage tanks; On or about October 23, 2024, after inspection and/or access openings roughly 2-feet to 2.5-feet in diameter were cut into the tops of exposed underground fuel storage tanks, classified as permit spaces, temporary railings, covers, or other barriers were not installed to protect employee(s) working near the openings from falling approximately 8-feet to the tank bottoms.
