The employer did not ensure that employee(s) engaged in roofing activities outside the warning line to perform work near the edge of the roof were protected from falling by a guardrail system, safety net systems, personal fall arrest systems, or a combination of warning line system and guardrail system, warning line system and safety net system, or warning line system and personal fall arrest system, or warning line system and safety monitoring system from a fall of approximately 19' to the ground below.  b)  778 Niagara Falls Boulevard, North Tonawanda NY - On or about 10/3/24, an employee was observed kneeling down working at the edge of the roof on the Kingston Avenue side of the building. The employer did not ensure that employee(s) engaged in roofing activities outside the warning line to perform work near the edge of the roof were protected from falling by a guardrail system, safety net systems, personal fall arrest systems, or a combination of warning line system and guardrail system, warning line system and safety net system, or warning line system and personal fall arrest system, or warning line system and safety monitoring system from a fall of approximately 19' to the ground below.
