29 CFR  1910.178(m)(5)(iii): Powered industrial truck(s), with the operator dismounted and within 25 feet of the truck still in his view, did not have the load engaging means fully lowered:  a) Worksite 1 Bennington Ave Freeport NY 11520 - An employee left a forklift unattended while the truck was in his view and did not have the load engaging means fully lowered. The operator left an employee on a personnel lift elevated approximately 8-feet above the ground; on or about 10/25/2024.   Note:  The employer is required to submit abatement certification for this item in accordance with 29 CFR 1903.19.

Note:  The employer is required to submit abatement certification for this item in accordance with 29 CFR 1903.19.

29 CFR  1910.178(a)(5):The powered industrial truck was equipped with front-end attachments other than factory installed attachments, however the employer did not request that the truck be marked to identify the attachments and show the approximate weight of the truck and attachment combination at maximum elevation with load laterally centered:  a) Worksite 1 Bennington Ave Freeport NY 11520 - Employees were using a Hangcha powered industrial truck with a front-end attachment that was not factory installed, and the forklift did not have placarding marked to identify the attachment and show the approximate weight of the truck and attachment combination at maximum elevation with load laterally centered, on or about 10/25/24.  Note:  The employer is required to submit abatement certification for this item in accordance with 29 CFR 1903.19.
