29 CFR 1910.212(a)(3)(ii): The point of operation of machines whose operation exposed an employee to injury, was not guarded:  a) Clean Room:  Production line 11 slitter:  The guard covering the guillotine on the slitter that cuts sheets of plastic to length had a three inch opening, exposing employees to an amputation hazard on or about 10/29/24.  b)  Clean Room, small bag sealer:  The opening of the heat sealing clamp on the small bag sealer was not guarded exposing employees to burns and amputations or about 10/29/24.  NOTE: IN ADDITION TO ABATEMENT CERTIFICATION, THE EMPLOYER IS REQUIRED TO SUBMIT ABATEMENT DOCUMENTATION FOR THIS ITEM. FAILURE TO COMPLY WILL RESULT IN AN ADDITIONAL PENALTY OF $1000.00 IN ACCORDANCE WITH 29 CFR 1903.19.

29 CFR  1910.147(c)(4)(i): The employer did not establish a program consisting of an energy control procedure, employee training and periodic inspections to ensure that before any employee performed any servicing or maintenance on a machine or equipment where the unexpected energizing, startup or release of stored energy could occur and cause injury, the machine or equipment was isolated from the energy source and rendered inoperative:  (a) Clean Room: A lockout tagout program was not established when an authorized employee performed preventive maintenance, cleaning and repairs of machinery including but not limited to slitters and sealers, used to cut and seal plastic sheeting on or about 10/29/24.
