29 CFR 1910.212(a)(3)(ii): The point of operation of machines whose operation exposed an employee to injury, was not guarded: a) Clean Room: Production line 11 slitter: The guard covering the guillotine on the slitter that cuts sheets of plastic to length had a three inch opening, exposing employees to an amputation hazard on or about 10/29/24. b) Clean Room, small bag sealer: The opening of the heat sealing clamp on the small bag sealer was not guarded exposing employees to burns and amputations or about 10/29/24. NOTE: IN ADDITION TO ABATEMENT CERTIFICATION, THE EMPLOYER IS REQUIRED TO SUBMIT ABATEMENT DOCUMENTATION FOR THIS ITEM. FAILURE TO COMPLY WILL RESULT IN AN ADDITIONAL PENALTY OF $1000.00 IN ACCORDANCE WITH 29 CFR 1903.19.
