29 CFR  1904.39(a)(1): Basic Requirement.  Within eight (8) hours after the death of any employee as a result of a work-related incident, you must report the fatality to the Occupational Safety and Health Administration (OSHA), U.S. Department of Labor.  The employer did not report to OSHA within 8 hours an employee fatality. The Fatality occurred on October 28,2024. This violative condition was documented on October 28, 2024, at or about 2440 19th Street, Omaha, Nebraska.
