29 CFR 1926.102(a)(1): Eye and face protective equipment were not used when machines or operations presented potential eye or face injury from physical, chemical, or radiation agents:  On or about October 29, 2024, Kelly's Roofing & Construction LLC was installing new shingles onto a roof of a residence at 910 Warrior Ridge in Warrenton, Missouri. The employer did not ensure eye protection was utilized by employees operating pneumatic nail guns.  Employees using nail guns without eye protection were exposed to struck-by hazards from broken wood, roofing materials, or nail debris.

Employees engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest systems:  On or about October 29, 2024, Kelly's Roofing & Construction LLC was installing new shingles onto a roof at 910 Warrior Ridge in Warrenton, Missouri. The employer did not ensure that fall protection was utilized by employees working on a roof greater than 10 feet above the lower level.  The employees on the roof were exposed to fall hazards.

The program had not enabled each employee to recognize the hazards of falling and did not train each employee in the procedures to be followed in order to minimize these hazards.  On or about October 29, 2024, Kelly's Roofing & Construction LLC did not provide training for employees who were exposed to fall hazards from roofing a residential building at 910 Warrior Ridge in Warrenton, Missouri. Employees were not trained to recognize the hazards of falling and the procedures to be followed in order to minimize fall hazards.
