29 CFR  1910.178(a)(6): The user shall see that all nameplates and markings are in place and are maintained in a legible condition.  a) On or about October 29, 2024, the employer did not ensure that all nameplates and markings were in place and maintained in a legible condition in that an unidentifiable counterweight riding propane fueled forklift was painted black, the nameplate had been removed, and the make and model of forklift could not be identified.

29 CFR  1910.303(b)(7)(iv): There shall be no damaged parts that may adversely affect safe operation or mechanical strength of the equipment, such as parts that are broken, bent, cut, or deteriorated by corrosion, chemical action, or overheating.   a) On or about November 13, 2024, the employer had not ensured that there were no damaged parts that may affect safe operation in that a metal pipe used as conduit for 110 volt live wires, exposed employees to electrical hazards when getting pallets.

29 CFR  1910.305(g)(1)(iv)(A): Unless specifically permitted otherwise in paragraph (g)(1)(ii) of this section, flexible cords and cables may not be used: As a substitute for the fixed wiring of a structure;   a) On or about November 13, 2024, the employer did not ensure that flexible cables were not used as a substitute for fixed wiring of a structure in that two extension cords were used to connect the Husky 15 amp 120 volt Air Compressor (Model C304H, FN  2004181) to the plug-in outlet.
