29 CFR 1926.501(b)(1): Each employee on a walking/working surface with an unprotected side or edge which was 6 feet (1.8 m) or more above a lower level was not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems. a) Worksite: 506 West Old Country Road, Hicksville, NY 11801 - Employees on the edge of a flat roof were installing a water resistant membrane on a commercial building and were not protected from falling approximately 11.5 ft to the ground below; on or about 10/31/24. Note: Because abatement of this violation is already documented in the case file, the employer need not submit certification or documentation of abatement for this violation as normally required by CFR 1903.19.

29 CFR 1926.1053(b)(1): Portable ladder used for access to an upper landing surface and the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface to which the ladder was used to gain access: a) Worksite: 506 West Old Country Road, Hicksville, NY 11801 - Employees were accessing the flat roof of a commercial building by using an extension ladder that was extended approximately 6 inches above the landing; on or about 10/31/24. Note: Because abatement of this violation is already documented in the case file, the employer need not submit certification or documentation of abatement for this violation as normally required by CFR 1903.19.
