29 CFR 1926.1053(b)(1): When portable ladders were used for access to an upper landing surface, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface to which the ladder was used to gain access; or, when such an extension was not possible because of the ladder's length, then the ladder was not secured at its top to a rigid support that would not deflect, and a grasping device, such as a grabrail, was not provided to assist employees in mounting and dismounting the ladder: Instances including, but not limited to, the following: a) Job site, 1323 Termon Avenue, Pittsburgh, PA: On or about November 5, 2024, during roofing related work on a roof on the side of the home, the employer did not ensure that the ladder used to get up/down extended at least 3 foot above the landing, exposing an employee to a fall hazard of about 12 feet.
