29 CFR 1926.501(b)(10): Each employee engaged in roofing activities on low-slope roofs, with unprotected sides and edges 6 feet (1.8 m) or more above lower levels was not protected from falling by guardrail systems, safety net systems, personal fall arrest systems, or a combination of warning line system and guardrail system, warning line system and safety net system, or warning line system and personal fall arrest system, or warning line system and safety monitoring system, or on roofs 50-feet (15.25 m) or less in width (see Appendix A to subpart M of this part), by the use of a safety monitoring system alone:   Instances including, but not limited to, the following:  a) Job site, 1323 Termon Avenue, Pittsburgh, PA: On or about November 5, 2024, during roofing work on the rear porch roof,  the employer did not ensure that an employee used appropriate fall protection systems/devices where exposed to falls of up to approximately 12 feet.

29 CFR  1926.1053(b)(1): When portable ladders were used for access to an upper landing surface, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface to which the ladder was used to gain access; or, when such an extension was not possible because of the ladder's length, then the ladder was not secured at its top to a rigid support that would  not deflect, and a grasping device, such as a grabrail, was not provided to assist employees in mounting and dismounting the ladder:   Instances including, but not limited to, the following:  a) Job site, 1323 Termon Avenue, Pittsburgh, PA: On or about November 5, 2024, during roofing related work on a roof on the side of the home, the employer did not ensure that the ladder used to get up/down extended at least 3 foot above the landing, exposing an employee to a fall hazard of about 12 feet.

29 CFR 1926.20(b)(2): The employer did not provide for frequent and regular inspections of the job sites, materials, and equipment to be made by competent persons designated by the employers:    a) Job site, 1323 Termon Avenue, Pittsburgh, PA: On or about November 5, 2024, during construction activities, the employer did not ensure that a system was in place to identify and correct safety issues at the job site. Deficiencies noted during the inspection included, but were not limited to, failing to use fall protection and failing to ensure ladders were properly set up at the job site.
