29 CFR 1926.102(a)(1): Eye and face protective equipment were not used when machines or operations presented potential eye or face injury:  a) New single-family residential structure located at 106 Creek View Dr. (Lot #6 Creekview Subdivision), Toccoa, GA 30577, Top plate and ground level: On or about 11/06/2024, the employer exposed employees to struck-by hazards while operating pneumatic nail guns, circular saw and hand tools, in that protective eyewear was not provided while engaged in framing activities.

29 CFR 1926.304(d): Portable, power-driven circular saw(s) were not equipped with a guard above and below the base plate or shoe:  a) New single-family residential structure located at 106 Creek View Dr. (Lot #6 Creekview Subdivision), Toccoa, GA 30577, Ground level: On or about 11/6/2024, and at times prior, the employer exposed employees to severe laceration/amputation hazards while operating a Makita 7 1/4-inch (Model 5007F) circular saw, in that the lower guard did not retract immediately after a cut was made as a chock was used to keep the lower guard in the open position.

29 CFR  1926.501(b)(13): Each employee engaged in residential construction activities 6 feet (1.8 m)  or more above lower levels was not protected by guardrail systems, safety net systems, personal fall arrest systems, or an alternative fall protection measure under another provision of paragraph 1926.501(b), nor did the employer demonstrate that it is infeasible or creates a greater hazard to use these systems and develop and implement a fall protection plan which meets the requirements of paragraph (k) of section 1926.502:   a) New single-family residential structure located at 106 Creek View Dr. (Lot #6 Creekview Subdivision), Toccoa, GA 30577, Top plate and ground level: On or about 11/06/2024, the employer exposed an employee to fall hazards of approximately 8-feet,1-inch to the lower ground level, in that an adequate fall protection system was not utilized while working form the top plate.
