29 CFR 1910.147(c)(4)(i):Procedures were not utilized for the control of potentially hazardous energy when employees were engaged in activities covered by this section: On or about October 29, 2024 at 1925 30th Street NE, Canton, OH 44705, the employer failed to ensure employees unjamming the cage conveyor system were protected from a caught in hazard by utilizing lockout/tagout procedures in that an employee attempted to unjam a cage utilizing their foot and got caught between the cage and the frame of the machine resulting in an injury.

29 CFR 1910.147(c)(7)(i)(A): Authorized employee(s) did not receive training in the recognition of applicable hazardous energy sources, the type and magnitude of the energy available in the workplace, and the methods and means necessary for energy isolation: On or about October 29, 2024 at 1925 30th Street NE, Canton, OH 44705, the employer failed to ensure employees unjamming the cage conveyor system were trained as authorized in lockout/tagout procedures in that an employee attempted to unjam a cage utilizing their foot and got caught between the cage and the frame of the machine resulting in an injury without being trained as authorized.
