29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks: (a) On or about October 20, 2024, and at times prior, the employer did not ensure employees were protected from amputation, caught-in and crush hazards, and puncture injuries while working in the car wash tunnel adjacent to the chain drive conveyors. An employee sustained serious injuries to his right foot when his foot made contact with the roller and chain drive through the floor opening the leads to the system.

29 CFR 1910.253(b)(4)(iii): Oxygen cylinders in storage were not separated from fuel-gas cylinders or combustible materials (especially oil or grease), a minimum distance of 20 feet (6.1 m) or by a noncombustible barrier at least 5 feet (1.5 m) high having a fire-resistance rating of at least one-half hour: (a) On or about October 28, 2024, and at times prior, the employer did not ensure that an Oxygen Compressed Cylinder UN 1072, Cat 2 was properly stored. The oxygen tank was directly adjacent to an Acetylene Dissolved cylinder, UN1001, Flammable Gas Cat 2, and greasy and oily material in the welding area exposing employees to fire and explosion hazards.

29 CFR 1910.303(g)(2)(i): Except as elsewhere required or permitted by Subpart S of Part 1910, live parts of electric equipment operating at 50 volts or more were not guarded against accidental contact by use of approved cabinets or other forms of approved enclosures or by any of the means identified in paragraphs (A), (B), (C), and (D) of 29 CFR 1910.303(g)(2)(i): (a) On or about October 28, 2024, and at times prior, the employer, did not ensure employees were safe from accidental contact with electrical live parts in the MCC/Main electrical room the cover to the electrical panel that provides service to the East Track B #26, 28, 30, 32 and West Track A #25, 27, 29, and 31 was opened exposing employees to 120 Volts. (b) On or about October 28, 2024, and at times prior, the employer, did not ensure employees were safe from accidental contact with electrical live parts in the MCC/Main electrical room the cover to the MCC-A identified as the ICS- WBC96, Tommy Wash Controller Panel was opened and unlocked exposing employees to 480 volts. (c) On or about October 28, 2024, and at times prior, the employer, did not ensure employees were safe from accidental contact with electrical live parts outside of the MCC room, the Purwater/Fresh-Water, PURCLEAN Panel-PW4005M250 (B) 160 GPM Purclean Reclaim Plus's panel was opened and unlocked exposing employees to 50 volts or more. (d) On or about October 28, 2024, and at times prior, the employer, did not ensure employees were safe from accidental contact with electrical live parts outside of the MCC room, the SOBRITE, Water Treatment Station that is connected to the back flow and pure pressure valves' electrical panel was opened and unlocked exposing employees to 50 Volts or more.
