(a) On or about November 7, 2024, located in bay 2, employees using forklifts to unload shipments and move material were exposed to struck-by hazards and tipping hazards, in that the CAT and TCM propane forklifts in Bay 2 did not have legible nameplates or markings indicating load capacities and other safety warnings.

29 CFR  1910.178(q)(7): Industrial trucks were not examined daily prior to use or were not taken out of service after the examination showed condition(s) adversely affecting the safety of the vehicle.  (a) On or about November 7, 2024, employees routinely operating the CAT and TCM propane forklifts in the facility were not required to conduct daily examinations to ensure no conditions were present that could affect the safe operation prior to use.

29 CFR  1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks.  (a) On or about November 7, 2024, located in bay 1, an Ellis Miter Band Saw had a section of its blade exposed that was outside the working portion of the blade, exposing employees to an amputation hazard.  (b) On or about November 7, 2024, located in bay 1, an Ellis Miter Band Saw did not have a guard installed to cover the portion of the blade outside the point of operation, exposing employees to an amputation hazard.  (c) On or about November 7, 2024, located in bay 2, an Accushear Shear had openings in guards that were large enough for employees to access the point of operation while the machine is in operation, exposing employees to an amputation hazard.  (d) On or about November 7, 2024, located in bay 4, the Makita Chop Saw had its spring loaded blade guard taped in its retracted position, exposing employees to an amputation hazard.
