29 CFR  1910.178(l)(1)(i): The employer did not ensure that each powered industrial truck operator was competent to operate a powered industrial truck safely, as demonstrated by the successful completion of the training and evaluation specified in this paragraph (l):   A) 226 Washington Ave Milltown, NJ 08850  - An employee was elevated 19 feet above the ground while standing on wooden pallet(s) on the forks of a JLG Skytrak Telehandler 8042. The employee was fastening exterior wood sheeting to the new building under construction without guardrail or personal fall arrest system(s) in use; exposed to hazard of falling 19 feet. Operator(s) of powered industrial trucks, including the JLG Skytrak Telehandler 8042, were not trained and evaluated in their safe operation.   Hazardous condition(s) were observed on or about October 31, 2024.

This condition exposed employee(s) to fall hazards.  Hazards observed on or about October 31, 2024.
