29 CFR  1926.453(b)(2)(v):A body belt was not worn and/or a lanyard was not attached to the boom or basket when working from an aerial lift.  On or about November 4, 2024, on the east side of the jobsite, an employee was assisting in the installation of roofing materials while utilizing an aerial lift (Genie S-45) without adequate fall protection, by not being anchored in the lift basket, approximately fourteen (14) ft. above ground.

29 CFR  1926.501(b)(1):Each employee on a walking/working surface with an unprotected side or edge which was 6 feet (1.8 m) or more above a lower level was not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems.  On or about November 4, 2024, an employee performed roofing work on a single-level commercial building six feet or more above the lower without fall protection. This employee was a subcontractor under the control of the Subcontractor  Precision Drywall Inc. The Foreman for Quality Drywall Inc was watching the employees as they worked without fall protection.
