Line 3 - On or about November 8, 2024, employees working near the B&K 3 TSR Supermill were exposed to crushing/amputation hazards. The shear was not effectively guarded. b. Line 1 - On or about November 8, 2024, employees working near the B&K DWR Supermill were exposed to crushing/amputation hazards. The shear was not effectively guarded. c. Line 1 - On or about November 8, 2024, employees working near the B&K DWR Supermill were exposed to being struck by metal studs exiting the roll-former. d. Line 2 - On or about November 8, 2024, employees working near the Dahlstrom rollforming machine were exposed to being struck by metal studs exiting the roll-former and cutting block. e. Line 2 - On or about November 8, 2024, employees working near the Dahlstrom rollforming machine were exposed to crushing/pinching hazards. The moving cutting block was not effectively guarded. f. Line 4 - On or about November 12, 2024, employees working near the B&K DWR Supermill were exposed to crushing/amputation hazards. The shear was not effectively guarded. g. Line 4 - On or about November 12, 2024, employees working near the B&K DWR Supermill were exposed to being struck by metal studs exiting the roll-former. h. Line 4 - On or about November 12, 2024, employees working near the B&K coil handler (uncoiler) were exposed to being struck by or caught by moving components of the uncoiler. i. Cut-to-length Line - On or about November 12, 2024, employees working near the shop made cutting shear were exposed to amputation/crushing hazards. j. Cut-to-length Line - On or about November 12, 2024, employees working near the Mecon Industries coil handler (uncoiler) were exposed to being struck by or caught by moving components of the uncoiler. k. Accessory Line - On or about November 12, 2024, employees working near the Tekfab machine were exposed to amputation/crushing hazards. The rollforming wheels were not effectively guarded. l. Accessory Line - On or about November 12, 2024, employees working near the coil handler (uncoiler) were exposed to being struck by or caught by moving components of the uncoiler.
