On or about November 12, 2024, at the jobsite located at 106 N. Riverside Dr, the employer allowed employees to use extension ladders to gain access to the roof and the rails of the ladders did not extend at least three feet above the roof landing to provide safe access exposing employees to a fall hazard of up to 35 feet to the ground below.
