29 CFR  1910.132(a): Protective equipment was not used when necessary, whenever hazards capable of causing injury and impairment were encountered:  (a) Facility: On or about November 9, 2024, an employee was operating a cutting torch, and he was not wearing any flame-retardant clothing. This employee was wearing red hooded sweatshirt (50% cotton and 50% polyester) and a pair of non-flame-retardant blue jeans at the time of the incident. This red hooded sweatshirt caught on fire and severely burned the employee.

On Saturday November 9, 2024, an employee was badly burned using a cutting torch to cut up an old crane cable.  The employee was not wearing any flame-retardant clothing.

29 CFR  1910.252(a)(2)(xiii): The employer did not recognize its responsibility for the safe usage of cutting or welding equipment on its property, in accordance with 29 CFR 1910.252 (a)(2)(xiii)(A) through 29 CFR 1910.252(a)(2)(xiii)(D):  (a)  Facility:  On or about November 9, 2024, the employer did not recognize the damaged hoses used to help cut the scrap steel crane cable.  These hoses (fuel gas and oxygen) had many cracks in them and need to be replaced.

29 CFR  1910.253(e)(5)(v): Hose showing leaks, burns, worn places, or other defects rendering it unfit for service were not repaired or replaced:  (a)  Facility:  On or about November 18, 2024, the fuel gas and oxygen hoses used on November 9, 2024, by an employee to cut through a scrap steel crane cable had many cracks in the hoses, especially near the connection to the fuel gas/oxygen supply.  These hoses need to be replaced with new ones.
