OSH ACT of 1970 Section (5)(a)(1):  The employer did not furnish employment and a place of employment which was free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to struck-by and crush-by hazards when utilizing damaged steel storage racks:  a)  Throughout the entire warehouse, located at 113 Industrial Blvd., Americus, GA, on or about November 13, 2024, the employer exposed employees to struck-by and crushing hazards in that, the employer did not display in one or more conspicuous locations a plaque to permanently and prominently display the permissible loading of the rack.  b)  Throughout the entire warehouse, located at 113 Industrial Blvd., Americus, GA, on or about November 13, 2024, the employer exposed employees to struck-by and crushing hazards in that, the employer did not ensure that the rack columns were anchored to the floor with anchor bolts adequate to resist all applicable forces.   c)  Throughout the entire warehouse, located at 113 Industrial Blvd., Americus, GA, on or about November 13, 2024, the employer exposed employees to struck-by and crushing hazards in that, the employer did not ensure that the affected areas of damaged racking was immediately isolated.  d)  Throughout the entire warehouse, located at 113 Industrial Blvd., Americus, GA, on or about November 13, 2024, the employer exposed employees to struck-by and crushing hazards in that, the employer did not have a storage rack design professional evaluate the damage, and unload, replace or repair if directed by the professional, any damaged columns, beams, or other structural components to restore the system to at least its original design capacity.
