29 CFR 1910.212(a)(1):One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks: On or about 11/12/20224, at the workplace located 1843 North Topping Ave., Kansas City, MO, employees were exposed to caught in and pinch point hazards from unguarded rotating and moving parts on the wire cutter machine. The employer did not ensure one or more methods of machine guarding was provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks . Machine guarding was deficient in the following areas of the machine, rotating belt roller, wire cutting arm pinch point, and rotating product feed roller.

29 CFR 1910.305(g)(2)(iii): Flexible cords and cables were not connected to devices and fittings so that strain relief was provided that would prevent pull from being directly transmitted to joints or terminal screws: On or about 11/12/20224, at the workplace located 1843 North Topping Ave., Kansas City, MO, employees were exposed to electrical shock hazards from a power cord used to power the wire cutter machine located in the production area. This cord had pulled away from the electrical box leaving the wires exposed.
