The SST40EIH-02 Versalift aerial device was not rated for material handling and the employer was using the aerial devices to lift utility lines of an unknown weight.

When employee and the conductive utility lines encroached the minimum approach distance for an unqualified person, multiple arch flashes occurred where the employee sustained serious injuries.
