29 CFR  1926.451(e)(1): A means of access for scaffold platforms more than 2 feet (0.6 m) above or below a point of access, such as portable ladders, hook-on ladders, attachable ladders, stair towers (scaffold stairways/towers), stairway-type ladders (such as ladder stands), ramps, walkways, integral prefabricated scaffold access, or direct access from another scaffold, structure, personnel hoist, or similar surface was not used.    a) Renpro LLC., 20 Frederick Street, Constantia, NY - At the South End of the Roof. On or about November 19, 2024, safe access and egress to/from the scaffold was not provided by the employer. Workers were exposed to fall hazards of greater than 6 feet while accessing and exiting the scaffold.

29 CFR  1926.501(b)(10): Each employee engaged in roofing activities on low-slope roofs, with unprotected sides and edges 6 feet (1.8 m) or more above lower levels, was not protected from falling by guardrail systems, safety net systems, personal fall arrest systems or a combination of warning line system and safety monitoring system.    a) Renpro LLC., 20 Frederick Street, Constantia, NY - At the South End of the Roof. On or about November 19, 2024, workers were exposed to fall hazards in excess of 10 feet while accessing the Werner ladder jack system off the dormer/entry of the building. The employer did not ensure workers were protected from fall hazards.

On or about November 15, 2024, workers were exposed to fall hazards in excess of 19 feet while performing roofing operations.   b) Renpro LLC., 20 Frederick Street, Constantia, NY - At the East End of the Roof. On or about November 15, 2023, workers were exposed to fall hazards in excess of 19 feet while performing roofing operations.   c) Renpro LLC., 20 Frederick Street, Constantia, NY - At the East End of the Roof. On or about November 19, 2024, workers were exposed to fall hazards in excess of 19 feet while performing roofing operations.
