The employer had not inspected for deformed, cracked or corroded members, loose bolts or rivets, cracked or worn sheaves and drums; worn, cracked or distorted parts such as pins bearings, shafts, gears, rollers, locking and clamping devices; checked for excessive wear on brake system parts, linings, pawls, and ratchets; inspected for inaccuracies in indicators, and for any pitting or deterioration of controller contactors, limit switches, and pushbutton stations.

29 CFR  1910.184(e)(1): Alloy steel sling(s) did not have permanently affixed durable identification stating size, grade, rated capacity, and reach:  On or about November 15, 2024, and at times before, the employer failed to ensure employees  were protected from crushing hazards by ensuring all alloy chain slings had a permanently affixed durable identification tag. A chain sling was observed attached to the overhead bridge crane and mold with no identification tag.
